Chronic kidney disease (CKD) is not a single condition — it's a final common pathway that many different diseases can lead to. Knowing the underlying cause helps your NephrocareMD team choose the most effective treatments, anticipate complications, and slow progression as much as possible.

In many cases, treating the root cause — controlling blood sugar, lowering blood pressure, managing an autoimmune condition — directly protects the kidneys from further damage.

Together, diabetes and high blood pressure account for the majority of CKD cases in adults. Both damage the kidneys through the same mechanism — injuring the tiny blood vessels that do the filtering work.

Diabetes

Persistently high blood glucose damages the delicate filtering vessels inside the kidneys. Over time, the filters become leaky — allowing proteins that should stay in the blood to spill into the urine. This condition, called diabetic kidney disease, progresses silently for years before symptoms appear.

As the damage accumulates, the kidneys lose their ability to remove waste and excess fluid. Without intervention, diabetic kidney disease can progress to kidney failure.

The good news: Tight blood sugar control, blood pressure management, and kidney-protective medications (ACE inhibitors, ARBs, SGLT2 inhibitors) can significantly slow or halt diabetic kidney disease progression.

High Blood Pressure

Elevated blood pressure puts mechanical stress on the blood vessels throughout the body — including those inside the kidneys. Over time, this damages the filtering structures, reducing their ability to remove waste and regulate fluid.

What makes this particularly dangerous is the feedback loop it creates: damaged kidneys struggle to regulate fluid balance, which raises blood pressure further — which causes more kidney damage. Breaking this cycle early is critical.

Other Causes of CKD in Adults

Polycystic Kidney Disease (PKD)

A genetic disorder in which fluid-filled cysts grow in the kidneys, gradually reducing their function over time.

Autoimmune Conditions

Diseases like lupus (systemic lupus erythematosus) can trigger immune attacks on kidney tissue, causing progressive damage.

Kidney Stones with Complications

Recurrent or obstructing kidney stones can damage kidney tissue and increase infection risk, contributing to CKD over time.

Renal Artery Stenosis

Narrowing of the arteries that supply the kidneys reduces blood flow and can lead to high blood pressure and kidney damage.

Severe Obesity

Excess body weight increases pressure on the kidneys and raises the risk of diabetes and hypertension — both leading causes of CKD.

Certain Medications

Some drugs — including certain pain relievers (NSAIDs), antibiotics, and chemotherapy agents — can be toxic to the kidneys when used long-term or in high doses.

Infections

Repeated or severe kidney infections (pyelonephritis) can scar kidney tissue. Some systemic infections also affect kidney function.

Hepatorenal & Cardiorenal Syndrome

Advanced liver disease or heart failure can impair kidney function — the organs are deeply interconnected.

Sickle Cell Disease

Abnormal red blood cells can block blood flow to the kidneys, causing repeated episodes of damage over time.

Rare Genetic Conditions

Conditions like Alport syndrome affect the kidney's structural proteins, leading to progressive kidney failure, often beginning in childhood or young adulthood.

Medications That Can Harm the Kidneys

Some commonly used medications can damage the kidneys — particularly with long-term or high-dose use. This is especially important for people who already have reduced kidney function.

NSAIDs (ibuprofen, naproxen)

Regular use of over-the-counter pain relievers like ibuprofen and naproxen reduces blood flow to the kidneys and can accelerate CKD progression. Always ask your provider before using them routinely.

Certain antibiotics

Some antibiotics — particularly aminoglycosides and vancomycin — can be toxic to the kidneys, especially at high doses or in patients with existing kidney disease.

Contrast dye (imaging)

Iodinated contrast used in CT scans can cause contrast-induced nephropathy in patients with reduced kidney function. Your provider will assess your kidney function before ordering contrast imaging.

Herbal supplements

Some herbal products contain compounds that are toxic to the kidneys. Always disclose all supplements to your NephrocareMD provider.
